- Your adjusted family net income is less than $90,000, based on your most recent tax return.

How To Apply — Step By Step
Applying for the Canadian Dental Care Plan (CDCP) is done directly through the Government of Canada—not through our clinic. Here’s a simple step-by-step guide to help you get started:
1. Check Your Eligibility
Before applying, make sure you meet the CDCP eligibility criteria outlined above.
2. Apply Online or by Phone
Visit canada.ca/dental to apply online, or call Service Canada at 1-800-277-9914 to apply by phone (automated system available 24/7).
3. Access Your Account
Sign in to your My Service Canada Account (MSCA) or create one if you don’t have it yet. Once logged in, you’ll find the CDCP application at the top of your dashboard.
4. Complete Your Application
Be ready to provide:
- Your Social Insurance Number (SIN)
- Date of birth
- Income details
- Information for any spouse or dependent children you’re including
5. Track Your Application
You can monitor your application status through your MSCA dashboard or by calling 1-833-537-4342.
Processing times are typically 4 to 6 weeks.
6. Receive Your Coverage Details
If approved, you’ll receive a welcome package from Sun Life by mail, including your member card and coverage start date.
7. Book Your Appointment
Once your coverage begins, book your appointment at Cruzado Dentures. Let us know you’re a CDCP member, and we’ll bill Sun Life directly on your behalf.
